What Are Medium-Chain Triglycerides?
Dietary fats are classified in part by chain length — the number of carbon atoms in the fatty acid. Long-chain fatty acids (12 or more carbons) make up the majority of fat in typical diets: olive oil, butter, nuts, and animal fats are predominantly long-chain. Medium-chain triglycerides (MCTs) are fats with 6 to 12 carbons, and their shorter chain length fundamentally changes how the body handles them.
The four MCTs are caprylic acid (C8), capric acid (C10), lauric acid (C12), and caproic acid (C6). Commercial MCT oil typically contains C8 and C10, which are the most efficiently converted to ketone bodies. Coconut oil is approximately 55% MCTs by weight, though much of this is lauric acid (C12), which behaves more like a long-chain fat metabolically.
How MCTs Are Metabolized Differently
Long-chain fats require bile acids for emulsification and specialized transport proteins called chylomicrons to travel through the lymphatic system before reaching the bloodstream. MCTs bypass this process entirely — they are absorbed directly into the portal vein and transported straight to the liver, where they are rapidly oxidized for energy or converted to ketone bodies.
This rapid hepatic metabolism has two practical consequences: MCTs produce ketones even in the presence of dietary carbohydrates (unlike dietary restriction-induced ketosis), and they are unlikely to be stored as body fat in the way long-chain fats can be.
What the Evidence Shows
Ketone Production and Cognitive Function
The most compelling application for MCT oil may be in neurological contexts. The brain can use ketone bodies as an alternative fuel when glucose availability is limited. This has generated interest in MCT supplementation for Alzheimer's disease, where glucose metabolism in the brain is impaired early in the disease process.
A 2018 study in the Journal of Alzheimer's Disease by Croteau and colleagues found that MCT supplementation significantly increased brain ketone metabolism in Alzheimer's patients compared to placebo. Studies by Henderson and colleagues showed cognitive improvements in a subset of Alzheimer's patients following MCT supplementation. The mechanism is plausible and this remains an active research area.
Weight Management
A 2015 meta-analysis by Mumme and Stonehouse reviewed 13 randomized controlled trials and found that MCT oil consumption modestly but significantly reduced body weight and waist circumference compared to long-chain triglyceride controls. The effect sizes were small — average weight loss of about 0.5 kilograms — but consistent.
The proposed mechanisms include enhanced satiety (via gut hormone release), higher diet-induced thermogenesis compared to long-chain fats, and the thermogenic effect of ketone body production.
Energy and Athletic Performance
MCTs are popular among endurance athletes and in ketogenic sports nutrition, with the logic that they provide rapidly available fuel that spares muscle glycogen. The evidence is mixed. Some studies show improved endurance performance with MCT supplementation, but gastrointestinal distress is a common and limiting side effect at doses above 30 grams per session. A gradual introduction protocol is essential for athletes considering MCT oil.
Appetite Suppression
Several studies have shown that MCT oil increases circulating levels of appetite-suppressing hormones (PYY and GLP-1) and reduces spontaneous caloric intake compared to long-chain fat equivalents. A study by St-Onge and Jones found that MCT consumption at breakfast reduced subsequent food intake at lunch by approximately 160 calories.
How to Use MCT Oil
MCT oil is a flavorless, colorless oil that can be added to coffee, smoothies, salad dressings, and protein shakes. It has a low smoke point and is not suitable for high-heat cooking.
Starting dose: 1 teaspoon (5 ml) daily to assess tolerance. Many people experience nausea, cramping, or diarrhea if they jump to higher doses immediately.
Maintenance dose: 1 to 3 tablespoons (15 to 45 ml) per day. Most research uses 15 to 30 grams.
Timing: Before exercise for performance, in the morning for appetite effects, or with a ketogenic meal to support ketone levels.
MCT oil powder is an alternative form that tends to produce less gastrointestinal distress, though it contains additional ingredients (typically tapioca starch) as a carrier.
Who Benefits Most — and Who Should Avoid It
Good candidates: People following ketogenic diets who want to maintain ketosis with more dietary flexibility, older adults with cognitive concerns working with a healthcare provider, individuals who tolerate it well and find it supports appetite management.
Use caution or avoid: People with liver disease (MCTs are metabolized almost exclusively in the liver), those prone to gastrointestinal issues, anyone managing cardiovascular risk factors without medical supervision.
MCT oil is a legitimate functional food with real but modest documented benefits. It is not a fat-burning supplement in any dramatic sense. Its most evidence-supported applications are cognitive health (particularly in neurological contexts) and as a tool for maintaining nutritional ketosis.
