Glycemic Load vs Glycemic Index: Which Matters More?
Understanding Glycemic Index
The glycemic index (GI) measures how quickly a standard portion of a carbohydrate food raises blood glucose. The reference food is pure glucose (GI = 100). A food's GI is measured by having test subjects consume a portion containing exactly 50 grams of digestible carbohydrate after an overnight fast, then measuring blood glucose at intervals over two hours.
Foods are ranked: - Low GI (55 or under): Minimal blood sugar impact - Medium GI (56-69): Moderate impact - High GI (70+): Rapid blood sugar rise
The GI concept is scientifically valid and practically useful as a rough guide to carbohydrate quality. However, it has a significant limitation: it measures the response to 50 grams of carbohydrate from a food, regardless of how much of that food you would actually eat.
The Problem the Glycemic Index Cannot Solve
Consider watermelon. Its GI is approximately 76 — high, in the same category as white bread. At first glance, this suggests watermelon spikes blood sugar similarly to white bread.
But a serving of 100 grams of watermelon contains only 7.6 grams of carbohydrate. To reach 50 grams of carbohydrate from watermelon, you would need to eat 660 grams (about 2.5 cups of diced watermelon) in a single sitting. That is not a typical serving. A typical 150g serving of watermelon contains about 11 grams of carbohydrate — far less than what the GI test measures.
This is where glycemic load (GL) corrects the GI's limitation.
Glycemic Load: The Practical Measure
Glycemic load = (GI × grams of carbohydrate per serving) / 100
For watermelon (GI = 76) in a 150g serving (11g carbs): GL = (76 × 11) / 100 = 8.4 — a low glycemic load.
For comparison, a standard 50g serving of white bread (GI = 75) with 25g carbs: GL = (75 × 25) / 100 = 18.8 — a high glycemic load.
Despite having similar GIs, a typical serving of watermelon has less than half the glycemic impact of a typical serving of white bread in real-world eating conditions.

Epidemiological Evidence for Glycemic Load
A 1997 study in JAMA by Salmeron and colleagues found that higher glycemic load diets were significantly associated with increased risk of type 2 diabetes in women, independent of total carbohydrate intake. This was an important early finding suggesting that the blood sugar impact of carbohydrate quality — not just quantity — matters for long-term metabolic health.
A 2008 meta-analysis by Barclay and colleagues, covering 37 prospective cohort studies, found that both high GI and high GL diets were associated with increased risk of type 2 diabetes, coronary heart disease, gallbladder disease, and breast cancer.
The 2015 International Scientific Consensus Summit concluded that both GI and GL are independently predictive of chronic disease risk and that dietary patterns favoring low GI and low GL foods should be recommended for disease prevention.
When GI Is Still Useful
Glycemic index remains useful in specific contexts:
Comparing similar foods: When portion sizes are similar (comparing two types of bread, two breakfast cereals, or two varieties of rice), GI provides a useful quality ranking.
Understanding processing effects: GI reveals how processing and cooking affect blood sugar impact. Parboiled rice has a lower GI than white rice; al dente pasta has a lower GI than overcooked pasta. These comparisons are meaningful.
Clinical settings: For individuals with diabetes or insulin resistance where precise glycemic management matters, both GI and GL provide useful information for food selection.
Practical Application
For everyday eating, glycemic load is the more actionable number. Here are GL values for common foods per typical serving:

The practical message from glycemic load data aligns with general dietary guidance: emphasize legumes, non-starchy vegetables, and minimally processed whole foods; limit refined grain products and sweetened beverages.
Most people do not need to calculate GL for every food — the pattern of low-GL eating is simply a whole-foods diet. GL calculations become most useful when comparing specific processed food choices or for clinical glycemic management.
